You remembered to insert the correct ipv4 in your portforwarding settings?

  4. Did you correctly portfoward the right port? 25565?

BT are little bitches for port forwarding.
Right, I'm on BT so I can help.
Firstly, check your port forwarding AGAIN as it likes to undo itself.
Now, if it's still up, restart your router. When you're up, check the port forwarding again. If it's good, you're good.
Now, I don't like Macs but here you go, flush your DNS cache in terminal:
OS X Mountain Lion or Lion
    sudo killall -HUP mDNSResponder
    Mac OS X v10.6
    sudo dscacheutil -flushcache
    Now, restart your "Computer" (sorry)
Start your server up again and watch your CPU max (again, I hate Apple)
Open up Minecraft and join your server, run around a little at 5FPS (don't hate me)
Tell your friend to join, it should have worked. If it didn't, buy a PC (joking) No seriously, if it didn't work, it's his internet.

Those screenshots made me think you were on a Mac. I'm glad to hear that you're not. :D
Alright, rinse and repeat. This time for the DNS cache, open up Command prompt by searching "cmd" then hitting enter, type this:
    ipconfig /flushdns
    Then retry.

SirRainbowGamer A static ip is an ip that doesn't change, if it changes then the portforward will mess up

Marsyoranges You do not need a static ip to host a server but it is prefered
SirRainbowGamer Currently who can log onto the server and with what type of address eg) IPv4, localhost, External Ip
